Possible causes :
1.Low level automatic transmission fluid . Check the dipstick.
2.Change due ATF . When did you change your atf ? Get a white tissue paper and let a drop of your atf .If the color turn brown atf should be change. Smell the use atf if smell burnt not good change it.

In my case (97 Accord), medyo mataas yung idling, kaya malakas yung shift shock (1.5k rpm at idle). When I had my mechanic adjust it (~900 rpm), nawala yung jerk everytime I shifted gears from a standstill.

That right when you have a high idling it causes shiftshock. You must have the correct adjustment of your idling.

So it does not require you to press the brakes...... mine does, and I think you should make it a habit since you should be at full stop when you shift to reverse since it will stress out your forward gears (D).
try having your kickdown cable checked or adjusted.
a good conditioned AT tranny should not be jerk or at least you should not feel it.
lets wait for the gurus to shed some light.
