I need a template to be able to notarize the medical certificate.

i need this coz my gf, filed for SSS sick leave for 2 months. but she was able to go back to work in just about 1.5 months. the company she's working for needs to have a notarized copy of the medical certificate that states that she is indeed fit to work.
