Normally, sa Finance Department (or its equivalent) nyo pwede makuha yan. Pwede na kasi ang substituted filing, kaya the employees need not personally file an ITR, yung company/employer nyo na ang nag-aasikaso nyan

ITR, now using BIR 2316, is a statement of the taxes witheld while you are employed and filed annually before 15th of April. As mentioned, substituted filing is now accepted.
Afaik, Your finance dept/HR should issue this to all employees before 15th of April.

Yes, pwede ka na mag-request ng ITR, since its a record of the income you earned from Jan - Dec 2006. In your case, July - Dec 2006 income lang ang mare-record.

One can only hope.
