it hasn't yet happened to my innova. but it did, to my other car.
with the belt in the correct tension, my guess is, the belt idler bearing.
to confirm, i try a few squirts of WD40 to it (the bearing! not the pulley mating surface!). if the sound disappears, then that's it.
the long-term solution is to replace it.
how much? i don't know.
